Low maternal serum levels of α-fetoprotein may indicate fetal Down syndrome. Elevated maternal serum levels suggest the presence of a fetal neural tube defect and certain other congenital abnormalities. Other causes of increased maternal serum α-fetoprotein levels include twins, gestational age greater than originally estimated, fetal edema, and fetal demise.
