Explanation
In the "locked-in" syndrome, patients with such tegmental-basal pontine strokes are unable to move anything but their eyes despite the preservation of normal consciousness. When the ischernic insult extends into the pontine, tegmental midline nuclei such as those of the fourth and sixth nerves results include dysconjugate eye movements. Other midline structures such as the parapontine reticular formation and the medial longitudinal fasciculus which integrate lateral eye movements may be involved with resulting paralysis or aberration of lateral gaze, including para-abducens palsy and internuclear ophthalmoplegia respectively
