The patient has a clear cell adenocarcinoma of the vagina which is observed in less than 0.14% of young women who were exposed in utero to DES. This estrogenic drug was formerly used to avert threatened abortion. The exposed daughters frequently (35-90% of cases) have vaginal adenosis, in which glandular columnar epithelium of Mullerian type either replaces or under grows the normal squamous epithelium of the vagina. The rare clear cell adenocarcinoma appears to arise from this abnormal glandular epithelium.
