The correct Answer is E

Thromboangitis Obliterans (Buerger’s Disease)

    • In men who are heavy smokers
    • Remission/relapses correlated with cessation/resumption of smoking
    • Pain on extremities (small arterioles and veins of the extremities)

    Etipathologic Correlates:

      • Tobacco products (CO2)—toxic to endothelial cells
      • CO affects O2 dissociation from hemoglobin—tissue hypoxia
      • Smoking affects catecholamine metabolism
      • Hypercoagulable state

    Morphology:

      • Small microabscesses within a thrombus

  • Inflammatory vascular disorder of small & medium arteries & veins, occlusive in nature
  • Primarily affects young men, linked to tobacco smoking & HLA-B5 / A9 antigens
  • Predominantly affects the foot
  • Progressively worsening ischaemic signs & symptoms are evident
  • Dx: Arteriography and excisional biopsy
  • Cessation of smoking crucial, Prognosis???
The coronary vessels may show other changes in addition to atherosclerosis: hyaline thickening in the arterioles occurs almost exlusively in heavy smokers; fibrous intimal thickening of the coronary arteries is also characteristic of „smoker´s heart“. Although smoking usually increases cardiac output, in patients with impaired cardiac function, a fall in cardiac output can result.
