For several years, the American College of Obstetricians and Gynecologists has encouraged a trial of labor in women with a single previous cesarean delivery. In a recent series of several hundred patients with more than one previous cesarean delivery there was no significant difference in the outcomes of a trial of labor in patients with multiple cesarean deliveries compared to a single cesarean delivery. Approximately 65% of women with multiple previous cesarean deliveries delivered vaginally, and catastrophic outcomes were rare, rising only slightly above the level seen in women with only one previous cesarean delivery.
