When the global network of developing country immunization programmes knownas the Expanded Programme on Immunization (EPI) was first established in the late1970’s, one of the greatest challenges was to ensure that the potency of vaccineswas maintained from the point of manufacture, usually in an industrialized country,to the point of administration in the field, under the harshest of climatic conditions.In addition to developing the technology needed to maintain this ‘cold chain’, theEPI also established a rigid set of parameters for the handling and storage ofvaccines. To maintain an operationally simple programme, at the peripheral levelthese parameters were defined by the most heat labile vaccine, oral polio vaccine(OPV).
Because it was not possible to precisely monitor the potency of anindividual vial, health workers would often have to destroy vaccines when theintegrity of the cold chain had been compromised.
