The Correct Answer is D

The emergence and spread of drug-resistant Streptococcus pneumoniae are causing changes in the suggested approach to treatment of otitis media in children. Over 50% of all isolates are no longer susceptible to penicillin in traditional dosages. Resistance rates to the macrolides have been reported to be similar to resistance rates for penicillin. In addition, the majority of pneumococci are no longer susceptible to oral beta-lactam antibiotics. Cefaclor and loracarbef appear to be substantially less active against this organism. Third-generation cephalosporins have less activity against gram-positive organisms than first or second-generation agents.
